In Accordance with House Bill 4224 (89th Regular Session) and Texas Health & Safety Code §181.105, the following information is provided to assist consumers in:
Requesting health care records
If you received counseling services from me during my employment as a community counselor at a local nonprofit agency, or if you are a third-party requesting records on behalf of a former client, please be advised that all records related to those services are the property of that agency and are maintained in accordance with its policies. Requests for records must be directed to the agency. For limited assistance or general inquiries, you may email me at kgutierrez@comunidadcounseling.com
Contacting the licensing authority
As a Licensed Professional Counselor – Associate in Texas, my license is regulated by the Texas Behavioral Health Executive Council (TBHEC). TBHEC can be contacted at Contact Us – Texas Behavioral Health Executive Council, and my license status may be verified via Online Search/Verify a License – Texas Behavioral Health Executive Council.
Filing a consumer complaint
Consumers may file a complaint with the Texas Office of the Attorney General – Consumer Protection Division at: https://www.texasattorneygeneral.gov/consumer-protection/file-consumer-complaint.
